| Change of Circumstances Forms | If your condition has significantly worsened, we can assist you with a change of circumstances. We can provide guidance and support to help you through this process. | £50 |
How this works?
- Please complete our referral form and select the appropriate option under Support Required.
- We will review your referral and send you a confirmation email if we are able to support your request. This email will include details on making a payment.
- Once payment has been received, one of our caseworkers will be in contact with you to discuss your case and provide you with your support.
- Once our support has been completed, we will contact you to provide some feedback on the support your received.
